To Whom it may concern;
It was brought to our attention that the fire labels at the pair of double-egress wood doors
for opening #G 1 02C were missing. Upon inspection of OUf files and records including the
factory order and acknowledgement, it was determined that this pair of doors was ordered
and fabricated to all fire door standards to warrant a 90-Minute fire rating in accordance
with Warnock-Hersey Intertek Testing Service. Further research with our factory
confirmed that fire labels were issued prior to shipping. Somehow, the labels had been
lost or removed.

Our visual inspection of the doors at the jobsite confirmed that they were fabricated as
ordered as we observed the "DFM90" stamping on the top ofthe doors which is done
dllring fabrication at the plant.

RE: Edgewood Early Childhood Ccnter and Westwood Gymnasium Addition
Following are the reslllts of the plan rcview for the Edgewood Early Childhood Center and
Westwood Gymnasiunl Addition. Our review was based on the Minnesota State Building Code
(MSBC) which adopted with amendments the 2000 International Building Code (IBC) with
handicap regulations of the Minnesota Accessibility Code Chapter 1341. Also requiremcnts of
the 2003 Minnesota Statc Fire Code (MSFC) which adopted with amendments the 2000
International Fire Code (IFC) and excerpts from the National Fire Protection Association
(NFPA).
1. Separate pcrmits for Building, Plumhing, BV AC, Sewer and Water, exit & emcrgency
lighting and Sprinklers required.
2. Electrical pennits will be required from the Minnesota State Board of Electricity.
3. Submit sprinkler plans designed hy liccnscd contractor/designcr. May be submitted at a
later date.
4. Elevator must be inspected by Statc of Minnesota Elevator Safety and the Kitchcn
inspected by thc Department of Health before occupancy.
5. Submit Energy Envelope Calculations for both buildings. MN Energy Codc 7670.0100.
6. Sht. AO.II : Providc plumbing fixture counts for both buildings.
7. Gymnasium a(idition: Provide an accessible route to accessible toilet facilities. The
accessible route to the primary functions area shall include toilet facilities and drinking
fountaills scrving the area of primary function. Exceptions: 1. The alterations to the path
of travel and the toilet rooms and drinking fountains serving the altercd area need 110t
exceed 20 percent of the cost of the alteration to the primary function area. MSBC
1341.0411, Subpart 2.
8. Sht AO.Il: Thc existing Westwood building allowable area calculation indicates that a
100 percent increase is used for an area increase. The gymnasium addition encroaches on
two sides of the existing building. Recalculate the area increases using the 1997 Uniform
Building Code (UBC) or the 2000 me. Include any assumed property lines, Fire wall

9. Sht AO.II: Indicate actual travel distances from existing Westwood classrooms through
the horizontal exit at the gymnasium addition.

width and travel distances with the gym divider closed.
11. Provide a draft cur:ain at elevator lobbies. MSBC 1307.0045 Subpart 3.
The plumbing, mechanical, electrical emergency lighting and alarm system plan review
comments are forthcoming. All items pertaining to the buildings must be addressed before a

1. Separate permits are required for the alarm system.
2. The 1999 ofNFPA 13 shall be the standard.
3. All materials shall be tested and installed per the listing.
4. All valves controlling water flow shall be monitored and secured including the PIV.
5. It is the responsibility of the fire sprinkler contractor to flush all main and branch lines.
6. Provide frost protection for all portions of the wet system.
7. Field verify all head locations.
8. Secure all valves against tampering. All valves in public areas shall be locked and protected.
9. Provide a water resistant system plan at the each sprinkler riser to include the following information.
a. Total site plan
b. All valves
c. Riser locations
d. Areas served by individual risers
e. All main and auxiliary drains
f. Location of inspector's test
g. Location of PIV valve.
10. For inspections call (952) 447-9850 give the project building permit number (05-1062)
10. Seal all penetrations in rated walls in an approved manor.
11. Provide protection for concealed combustible areas.
