REQUIREMENTS
1. No work under this application is to be started until application is approved and the permit is issued.
2. Where work on traveled roadway is necessary, traffic must be protected and flags, signs, safety cones and proper
barricades must be placed in accordance with safety procedures.
3. No foreign material such as dirt, gravel, or bituminous material shall be left or deposited on the road during the
construction of driveway.
4. Road must be swept clean after work is completed.
5. Prior to placement of concrete or bituminous surfacing, the applicant shall notify the Engineering Department that the
work has been completed and is ready for final inspection.
6. No changes or alterations in entrances may be made at any time without written Dermission from the Engineering
Department or inspector.
IMPORTANT: Before starting work. . .
The Applicant shall call Gopher State One Call (1-800-252-1166) 48 hours prior to starting work. Applicant must call the
Engineering Department (952) 447-9830 for inspection 24 hours prior to time of inspection.
When widening or constructing new driveway approaches a $10.00 fee is required at the time of application and a sketch of the
property depicting present and proposed driveways in relation to the property line.

As provide in Ordinance Numbers 1107.205 & 1107.206> I, We the undersigned, herewith make application
for permission to construct the driveway at the above location. said driveway to be constructed to conform
with the standards of the City of Prior Lake and to any special provisions included in the permit. It is agreed
that all work will be done to the satisfaction of the Engineering Department. It is further agreed that no work
in connection with this application will be started until the application . pprov and t . ued.

AFTER SPEAKING WITH JOHN THIS MORNING' HAVE REVISED MY ESTIMATE TO REFLECT A 23 FOOT
WIDE BITUMINOUS MAT VERSUS THE 22' AS SHOWN ON THE PLAN THAT I HAVE.

GUYS - THIS IS WHAT I BEUEVE WILL BE A STABLE lONG TERM SOLUTION FOR THE CHURCH FOR
MODERATE ,"OAD TRAFFIC (I.E. BUSES ETC )
THERE ARE 3 AREAS WHERE THE COSTS COULD BE ADJUSTED:
GRADING; THE NUMBER ABOVE IS A NOT TO EXCEED NUMBER INVO\.VING STRIPING UP TO
l' OF TOPSOIL FROM THE PROPOSEO DRIVEWAY AREA BRINGING IN UP TO 6" OF BORROW MATERIAL
TO CONSTRUCT THE ROADWAY. ONCE THe AREA IS STAKED WITH CUTS/FILLS THIS CAN BE VERIFIED
BUT I THINK I AM REALLY CLOSE LOOKING AT THE PROPOSED ALIGNMENT. .
GEOTEXTILE FABRIC: I HAVE INCLUDED THIS BECAUSE OF THE SOIL CONDITIONS THAT ARE ON THE
SITE - HEAVY CLAY - TIE GEOTEXTU..E FABRIC KEEPS THE GRAVEL AM> THE ClAY SEPARATE INSURING
A FIRM GRAVEL BASE FOR ntE BrTUMlNOUS TO SIT ON. l11IS CAN BE EUMINATED BUT I THINK FOR THE
MINOR COST IT IS BEST TO GO WITH IT.

NOT KNOWING HOW THE SlOPES WIll WORK TO MEET THE EXISTING lAND
I GUESSED THAT WE WilL BE OISTURBING UP TO 15' EACH SIDE OF THE
PROPOSED DRIVEWAY.. AGAIN I THINK IT WILL BE LESS BUT I WOULD RATHER
BE CONSERVATIVE AT THIS POINT.

Re: Friendship Church Driveway
We acknowledge that we are working with Tollefson on two easements with our property and we
are having a driveway extended from Thornton Drive to our parking lot for a second access point
from our property.
We understand that in the future if 176111 street gels extended we will still be responsible for our
"fair market assessmenr of that future road improvement.
We also understand that if Friendship Church does any major renovation or expansion project. at
that time we might be required to connect up to city sewer and water and pay all applicable trunk
fees at that time in order to get the project approved. (Current estimate for hook up withafees is
approximately $232,000.) .-'-
We also acknowledge that we would have one year to connect to city SEMD and water when
service lines gets connected out 10 our property. Right now only the main service lines will be
extended to our property from Tollefson. This will not start the one year timeline and in the future
we wiD have the opportunity to connect to city sewer and water when we choose to extended the
service lines.
Attached is Permit for driveway and map plans from Tollefson.
