The following modifications, deletions and additions shall append and become an
integral part of the Contract Documents issued for this project, including Addendum
#1. Where any portion of the Contract Documents is modified, deleted, added or
voided, the unmodified potion of the Work shall remain in effect. References listed
herein shall be used for reference purposes only. The onus is on all contractors and
sub-contractors/suppliers to cross-reference all items listed and determine for
themselves the Work effected by the addendum items.
Attached to this addendum is the Bidders List through 10/25/95
CHANGES TO THE BIDDING REQUlREMEN1S AND CONTRACT FORMS
1. J\.ttached is a revised Bid Form for General Contractors, which includes two
additional Alternates. Please use this form when submitting your bids.
CHANGES TO THE SPECIFICA nON
1. Section 01010, Summary of Work: Contractor shall allow $5,000.00 for all
permits and SAC & WAC charges. Any adjustments to this shall be made on
a direct cost basis, with the credit going back to the city or an add being
added to the contract via a Change Order. City shall verify this number by
the start of construction.
2. Section 01010, Summary of Work: Include under mechanical work a 30 gallon
electric water heater that is to be located between the two sinks. Verify height
in field. Provide mixing valve @ restroom fixtures (allowing the city to mix

water at the lavatories as necessary). Install valve under restroom countertop.
Mechanical: also include any back flow valves and check valves as required
to effectively operate all domestic water-related items.
Electrical: NSP shall provide new 15 KV A transformer with meter adjacent to
the existing transformer and new building (by Owner /NSP). Contractor shall
make connection from interior panel to this transformer for the 110/120 V
service: include all underground work and materials necessary. Also provide
a connection for the water heater.
3. Section 01020, Allowances: Include Allowance #2, as noted above, which is the
$5,000.00 in city fees/charges.
4. Section 01030, Alternates: Two additional alternates have been added. Refer
to the revised Bid Form (attached) for additional information.
5. Section 02200, Earthwork: Include the following information:
"Contractor shall assume that the soils materials at the building site are
suitable for backfilling, and have adequate bearing capacity. Under the base
bid the contractor shall excavate down to the specified footing elevation, and
place footings directly on what is presumed to be virgin soil. There are no
soil borings, but the city is satisfied with the underground and soil conditions
at the site. During the work, if contractor uncovers any conditions for
concern, advise the Architect/City of Prior Lake and the remedies/changes
shall be discussed on site. If additional imported material is required,
contractor shall provide on a unit/price basis, and the contract adjusted
accordingly.
Prior to work being performed by the Contractor, the city shall demolish the
existing toilet building, relocate the small concession stand and bleachers and
identify all underground utilities. The city shall remove existing footings,
thus leaving the open trenches, from which the General Contractor shall
p'roceed with the work specified.
The only imported material shall be a 6" layer of specified granular material
for use under the slabs-on-grade. No other base bid, imported material is
required, other than that identified above. Any excess material shall remain
in a pile on site adjacent to the new building, for removal/use by the city.
The city shall provide a new curb box at the 2" water line location,
approximately 5' from the new building southwest corner.

6. Section 07630, Standing Seam Roofing: the system used on the project shall be
equal to the ColorKlad System 1, with narrow profile 1" tall rib, 18" nominal
width & hold-down cleat. All spec-listed or approved manufacturers shall be
equal to this system or will not be acceptable. Delete reference to 20" panel
width.
7. ROOM FINISH SCHEDULE (and related): corridor walls (room 100) shall be
standard grey block (unpainted under both base and alternate bid) with
natural cement. Note that base bid exterior walls are rock-face block of two
colors, with an alternate for natural grey rock-faced block (as identified in
attached revised Bid Form).
CHANGES TO THE DRAWINGS
1. Sheet Al.l: At door 101, material noted @ corridor as "er" is to be Concrete.
2. Sheet Al.l: At the bottom left comer, the beam shown as 13 feet long shall be
6'-6" long to match the opposite comer.
3. Sheet Al.6: At the recessed storage shelving area, provide FRP panels in lie
oj the epoxy paint as noted.
4. Sheet Al.6: Delete reference to clear sealing the non-exposed trusses above the
restrooms.
5. Sheet Al.6: height of Pressbox is not indicated. Provide number of courses
of block as shown.
6. Sheet Al.7, detai12: Delete reference to epoxy paint at the soffit and provide
stain at the plywood.

AGREEMENT:
Deduct: Delete the three compartment sink and hand sink from the
work and provide rough-in.

The bidder agrees this Bid shall be good and may not be withdrawn for a period of thirty
(30) days after the scheduled time and date for receiving bids. A bid bond is included with
this bid form and shall be accepted by the Owner in the event of a withdrawal or forfeiture
by the Bidder.
The bidder understands the Owner reserves the right to accept or reject any or all bids or
alternates and to waive informalities in bids received and monitor discrepancies in bidding
procedures. If the Owner or Architect, in reviewing this Bid Form, discovers that the
information provided has been falsified or exaggerated, or if the Contractor does not meet
the pre-established criteria, same Contractor shall forfeit his/her right to the contract.

The bidder also agrees, if awarded the contract, to complete his/her work in-accordance with
the tentative Owner's Milestone Dates listed in Section 01010, Summary of Work with a final
substantial completion date after the Notice to Proceed.

BID GUARANTEE:
The undersigned agrees that the Owner shall have the right to retain this bid for a period of
thirty (30) days from the date and time of receiving bids and guarantee through a bid bond
in the amount of five percent (5%) for the same thirty (30) day period.
BID ACCEPTANCE
If written notice of the acceptance of this bid is mailed, telegraphed or delivered to the
undersigned within the time noted herein, after the date of the opening of bids, or any time
thereafter before this bid is withdrawn, the undersigned agrees that he/she will execute a
construction contract (AlA document A101, 1987 edition) in accordance with the bid as
accepted and will furnish contract security in the form of Performance and Material and
Labor-Payment Bonds in such form, amount and with such surety or sureties as the Owner
may approve. The cost of the Performance and Material and Labor Payment Bonds is
included in the base bid contract sum.
It is understood and agreed that the Owner reserves the right to award the contract to his/
her best interests, to reject any or all bids, to waive any informalities in bidding, and to hold
all bids for the bid guarantee period.
