Summary of Work:
On July 29, 1994, I visited the site of the building shell
design for Prior Lake Commercial Development in Prior Lake,
Minnesota, to provide Special Inspection Services for
reinforcing placement in concrete masonry. At the time of my
observation, the Contractor was off site. The following is a
summary of my observations.
Field Observation Notes:
I. General status of work
A. Footings are installed with the exception of interior pad
footings.
B. Exterior masonry walls and interior masonry shear walls are
installed up to grade.
C. Walls were not backfilled.
II. Ge:,eral obs2rvatians.
A. Shear wall on Grid 3-
At the east end two dowels are installed for corefilling as
required. However, one of the dowels falls within the
column pier adjacent to the end of the wall and is
separated from the end corefill by the face shell of the
block wall.
To remedy this, the Contractor should knock out the end
face shell of the wall so that the 8" corefill can be
poured integral with the column pier. �vo verticals should
be installed in the 8" corefill at the end of this wall.

B. Shear Wall on Grid C-
At the north end, two (2) dowels for 8" corefill with 2-#5
verticals exist as required. However, the core appears
very "tight" due to the way block was layed-up in this
Iocation.
The Contractor should provide a cleanout at the bottom of
this corefill to insure concrete gets installed full height
of this grout lift.
C. 24" corefill for beam bearing at grid location 1B-
An additional vertical bar and corefill is required at the
east end of the existing dowels. I marked this location on
the face of block in the field.
D. 24" corefill for beam bearing at grid location 1D-
An additional vertical bar and core=ill i� re�uired at the
east end of the existing dowels. I marked this location on
the face of block in the field.
E. Column piers along the east and south walls require #3 ties
at 10" on center. Ties must be added to achieve the
required spacing.
F. The bond beam along the top of 12" walls on the east and
south sides of the building must run continuous thru
vertical tube locations and column piers. The Contractor
will need to knock out some face shells in order to achieve
this.
G. The areas the Contractor appears to have provided for
vertical tubes as required by Detail R/Al2 along the east
and south walls generally appeared satisfactory with a
couple of exceptions. Additional tubes are required near
the column piers at grid locations 3F and 6B.
H. The column footing at Grid location 2C is installed with
top of footing at 97'-4" and anchor bolts directly in the
footing in lieu of a pier as i:.ter�ded. Thess a:�chor �olts
should be abandoned, and a pier installed per Detail B/Al2.
The Contractor will need to drill and grout dowels to
footing and install reinforcing as detailed. Also, add new
anchor bolts at top of pier.

INTRODUCTION
This report concerns the soil compaction testing we performed for the Prior Lake Center Project in Prior Lake,
Minnes�ta. Dale Allen of Yaggy-Colby Associates authorized this work on July 14, 1994.
SCOPE OF WORK
The scope of our recent work consisted of the following activities:
l. Performance of forty-two field in-place soil density tests on an intermittent basis as requested by Yaggy-
Colby Associates.
2. Dztermination of the compaction characteristics of each type of soil encountered in aur field density tests.
3. Comparison of the test results to the compaction ievels requirzd by the project specifications.
TEST I�IETHODS AN'D RESULTS
Details of our recent testing are included on the attached data sheets. The field density tests and determinations
of compaction characteristics were performed in general accordance with the ASTM procedures indicated on
the data she�ts.
Review of the test results indicates compaction meets project specifications in all test areas except # 10.

Dear Mr. Nielson:
We recently observed the excavation observation to remove fill and unsuitable soils from
within the proposed building area. The geotechnical report dated March 1, 1994, prepared by
Yaggy Colby Associates, indicated unsuitable fill soils ranging in depth from about 7.5-feet to
20-feet within the proposed building area. The fill soils were judged unsuitable for foundation
support. The report recommended that the fill soils be excavated, unsuitable soils removed, and
the clean fill soils be placed and compacted to support building foundations and floor slabs.
The excavation was completed beginning on July 13, 1994 and completed on July 18, 1994. A
geotechnical engineer was on site on July 13th, 14th, and 15th to observe the excavation and
check the excavation bottom, and judge the suitability of the excavated soils for reuse as
compacted fill. Hand auger borings were taken at random in the bottom of the excavation to
check that all unsuitable fill soils were removed from below the building area. Also, the
excavation was checked to see that adequate oversizing was provided in the bottom of the
excavation. The oversizing is required to assure that lateral support is provided for the fill
placed below foundations. Excavation depths ranged from about 5-feet at the far northwest
corner of the building to about 12-feet in the southeast building corner. The excavated fill soils
consisted mostly of a mixture of silty sands, clayey sands, and sandy lean clay. Some buried
bituminous and concrete debris were encountered. A couple of tree stumps and some debris
were also encountered. The majority of the excavated fill soils were suitable and used for
compacted backfill in the building area.

A total of 42 compaction tests were completed on fill placed in the building azea. These tests
were completed by Huntingdon Engineering and Environmental. Copies of test reports are
attached. Test results indicated that the fill was compacted to an excess of the required ninety-
five percent (95%) of Standard Proctor Density (ASTM-D698).
It is our opinion that all unsuitable soils were removed from within the building area and that
the compacted fill placed is suitable for spread footing foundations designed for net foundation
bearing pressures of up to 2,500 pounds per square foot (psfl.

A. All beams and columns were in place.
B. Light gauge framing from the roof to the top of the windows
in the south and east wall were in place and angles to
joists were in place along the east wall.
C. The roof deck was in place.
II. General observations.
A. All beam and column connections appeared to be in general
conformance with the Plans with the exception of Grid B-5;
bolts in the beam to column connection were not observed at
the time of this inspection at this location. These should
be installed.
B. Connections of the light gauge members appeared to be in
general conformance of the Plans. The bracing angles as
per Detail P/Al2 connecting back to the joists were welded
to the bottom chord rather than the top chord of the joists
at panel points as specified. To correct this, bracing
will be added from the bottom chord at the connection
points to the top chord and additional bracing between
joists ( see attached Detail).

On November 18, 1994, at the request of Bob Copeland, I visited
the site of the Prior Lake Commercial Development in Prior Lake,
Minnesota, to review field welding and general installation of
wall bracing. My primary purpose was to review wall bracing
which was revised twice previously.
While on site, I met with Jumping Johnny, Project Superintendent
for Copeland Mithun, and we walked the site reviewing each joist
condition. The following is a summary of my observations.
Field Observation Notes:
I. General status of work
The building is enclosed and installation of interior partitions
and mechanical work is in progress. Wall bracing to roof joists
had been revised a second time, supposedly in accordance with
the sketches issued 10/25/94.
II. General Observations
A. I r_eviewed the beam to column weld (installed in lieu of
A325 bolts due to alignment problems) at grid location 6B.
This installation appears satisfactory.
B. Wall bracing: �
1. At the time of this visit, Johnny was aware that the �
bracing installation did not comply with the sketches !
submitted in our last Inspection Report dated
10/25/94.
2. If possible, we want to work with the existing bracing
and add material as required to achieve an acceptable
solution. We reviewed each brace and, where required,
developed solutions; nearly all braces require some
retrofit. Retrofit requirements are summarized on the
attached sheet 1 of 2 which was put together while on
site.

3. The horizontal tubes, along the exterior wall to which
