excited about having the final day of the tournament at Lakefront Park. This
tournament will raise several thousand dollars for the Cystic Fibrosis Foundation.
Additionally, the all day event at Lakefront Park will provide entertainment and
fun for our residents.
Representatives from Waste Management, KFAN, Cystic Fibrosis Foundation
and Extreme Bass will update the City Council with a 10 to 15 minute
presentation on the activities slated for June 16th in Lakefront Park.

1. Any moving or Extream Rules violation on the lake could be reason to be DISQUALIFIED
2. Use common sense and courtesy when approaching other boats on a spot.
( a good cast distance from both boats is a good rule of thumb.)
3. NO alcohol or illegal drugs during tournament hours.
4. Life vests MUST be worn when the large motor is running. Engine shut off cord must be worn.
5. 12 inch min.
6. Any fish(s) under 12 inches will result in a loss of the short fish(s) and largest fish. *Note: Once your bag is given to the weigh
master it becomes offical, so remember to measure your fish before you weigh them.
7. Liability release: entering any of the Extream Bass Tournaments you release all agents, officers, employees, sponsors and
government agencies from any and all damage claims and or costs, relating to injury to any person or property which may sustain
or cause in connection with a tournament. You are required to have full insurance. You are required to have and submit a "copy"
of a current boating liability policy. Please include a photo-copy of your policy with your application and entry fee.
8. Any fish that appears to have excess stress or is not in healthy condition will not be allowed to weigh in.
9. No live bait and no trolling.
10. All state fishing regulations apply.
11. The Extream Bass Tournament Directors will make the final judgment on all violations, if you have a complaint of a rule
infraction you must first take a rules form and write the complaint out. Then sign it and give it to the rules committee with a
$50.00 deposit which will be returned if it is upheld.
12. No more than 9 fish allowed in live well.
13. You will only be allowed to weigh in 8 fish for the weekend tournaments. Any bags the exceed this limit will be disqualified.
14. **No Passing** through marked "No Wake" areas
15. 4 oz. penalty for dead fish weighed in.
16. One member of each team must be 18 years of age.
17. Each team must be a paid member of Extream Bass. There is a $300.00 entry fee that must be paid no later that May 26th 200 I.
18. Permitted fishing methods: All bass must be caught live and in a conventional manner. Only artificial lures may be used with the
exception of pork trailers. Trolling as a method of fishing is prohibited.
19. All angling must be conducted from the boat. At no time may a contestant leave the boat to land a fish or to make the boat more
accessible to fishing waters.
20. TIES: In case of a ties, Positions will be determined by: I st tie breaker-number of fish alive. 2nd tie breaker-total fish. 3rd tie
breaker-big fish. 4th tie breaker-coin toss.
21. MANDATORY TOURNAMENT MEETING - WEDNESDAY - 4:00 P.M. GANDER MOUNTAIN, BURNSVILLE.

1. I certify that the watercraft in which I will participate in this special event will not have been in any of the listed infested waters for at least thirty (30) days
prior to participation in the event; or, if the watercraft has been in such infested waters within such thirty (30) day period, it will have been completely dry
(including all live wells, bilges, and other parts of the watercraft which ar capable of holding water) for a period of at least five (5) consecutive days prior to par-
ticipation in the event. By my signature below 1 acknowledge that I understand: I) that a false certification is a crime under the ordinances of the Lake
Minnetonka Conservation District (LMCD); and 2) that if the watercraft identified below is placed in infested waters within thirty (30) days prior to launching
into Lake Minnetonka and is not completely dry for five (5) consecutive days during this period, I will be required by law to have the watercraft inspected and
cleaned by personnel approved by the LMCD.

Extream memberships is required to fish all Extream tournaments, membership lasts one year.
LIABILITY RELEASE.
I as a Extream Bass member agree to the following terms and conditions:
1. I will assume all risk of injury and or death which may result from fishing in the Extream Bass tournament series.
2. I will release all Extream Bass Sponsors, Directors and employees from all liabilities from accident, injury or death that may have
resulted in the Extream Bass tournament series.
3. I have read and understood all of the Extream Bass rules & will abide by all state & local laws, ordinances & Extream Bass rules.
4. I will allow the tournament to use all pictures, Videos and names for the news media or advertising of the tournament without any
compensation to me.

This is a 2 person team event with a $300 entry fee.

WILL GO TO JEREMY FLICEK
Jeremy Flicek was seriously injured in a car accident December
12,2001. He is still in serious condition at North Memorial
Hospital. A "Jeremy M. Flicek Benefit Fund" ho.s been setup for to
help cover medical expenses at Wells Fargo Bank. All ofus at
EXtream Bass send our prayers and thought to Jeremy and his
family. We all hope Jeremy recovers soon.
SSO from each entry fee will be donated to Cystic Fibrosis
