LLC Water Tower Specialist / Antenna, Evaluations, Inspection P.O. Box 157 Osseo, WI 54758 715 -533 -8686 June 5, 2012 By E -Mail only Bob Hutchins Building Official City of Prior Lake 4646 Dakota Street SE Prior Lake, MN 55372 RE: Initial Review of the Proposed Antenna Installation on the Tower Street Water Tower in the City of Prior Lake, Minnesota AT &T Site: MLPSMN3434 BSI Project WI 1010 Dear Mr. Hutchins: BSI has completed an initial review of the drawings for the above referenced antenna installation on the Tower Street water tower. It appears the project consists of installing underground conduits by open trenching, fiber optic lines, and equipment cabinets for connections with AT &T equipment within the AT &T equipment shelter. It does not appear that the installation will affect the actual water tower in any manner. As such, the only comments that BSI has are: 1. The City of Prior Lake should verify the proposed locations of lease area(s) and easements are accurate per both verbal instructions and the signed lease agreement(s). 2. Drawing T -1 under Site Information, please add the following under site contact Bob Hutchins 4646 Dakota Street Prior Lake, MN 55372 (952) 447 -9851 3. Drawing C -1 Site Plan: This drawing indicates there will be approximately 275 LF of 4" Schedule 40 PVC conduit required for fiber optic cable. AT &T will need to provide a trench detail for the underground utilities. Please provide all existing AT &T utility locations. The City would prefer that this installation be completed by directional boring with a minimal amount of trenching. Please clarify what portions of the installation will be hand dug and what areas are directional bored. 4. Drawing C -1 Site plan: The drawings do not show existing trees and shrubs that exist on the site and in particular near where the work will take place. Please revise the drawings to show the existing vegetation around the site. 5. Drawing GN- 1(General Notes) Items 24 through 28, incorporate site restoration but are lacking detail. Please add the following additional site restoration requirements Landscaping and Site Restoration: • Furnish, install and maintain as shown or required within the construction drawings and specifications. Protection: • Protect seeded areas from erosion by spreading straw to a uniform loose depth of '/2 inch. Stake and tie down as required. Use of erosion control, mesh or mulch will be an acceptable alternative. • All trees placed in conjunction with a landscape plan will be wrapped, tied down with hose protected wire, and secured to 2 inch by 2 inch by 4 foot steel angle extending two feet into the ground on four sides of the tree. • All exposed areas shall be protected against washouts and soil erosion. Installation: • Prior to seed application, remove all rocks and debris. Seed and fertilizer shall be applied to surface conditions which will encourage rooting. Rake areas to be seeded to even the surface and loosen the soil. • Sow seed in two directions in twice the quantity recommended by the seed producer. • It is the contractor's responsibility to ensure growth of seeded and landscaped areas by watering up to the point of release of the contract. Continue to rework bare areas until complete coverage is obtained. 6. Please provide a set of AS built drawings to the owner at the completion of the project. These recommendations should be incorporated into the construction drawings and an updated copy provided for our review, in addition to the one provided to the City of Prior Lake. Following final review and approval, a preconstruction conference with AT &T, their contractor and subcontractor(s), the City of Prior Lake, and BSI should be scheduled to discuss the construction. It should be relayed to the contractor that BSI will be performing limited site inspections on behalf of the owner and it is the job of the general contractor to schedule these inspections. These will be discussed at the preconstruction meeting. If you have any questions, feel free to contact me at 715 -533 -8686. Sincerely, /1. C, Kelly C.
