DISCUSSION: Introduction
The purpose of this agenda item is to consider approval of a massage therapy li-
cense for Paula Pentico.

Massage therapists are required to be licensed in the City of Prior Lake if they are
not working directly under a state licensed practitioner such as a physical thera-
pists or a chiropractor.
Current Circumstances
Paula Pentico, 4000 West 88 St #305, Bloomington MN 55437 has applied for a
massage therapy license in order to practice massage therapy in Prior Lake. She
is planning to practice at Mary on Main Salon & Spa, 16154 Main Avenue #110A.
Ms. Pentico has provided the city with the required proof of education and training.
Massage therapy licenses are issued on a calendar year, which would make this
license valid until December 31, 2013, at which time it would be subject to renewal
along with all other licensed massage therapists in Prior Lake.
Conclusion
The Prior Lake Police Department has completed its investigation of this applicant
to ensure that all issues concerning public safety and compliance have been met.
FINANCIAL Ms. Pentico has paid the $100.00 investigation fee and the $100.00 annual license
IMPACT: fee.
ALTERNATIVES: 1. Approve the massage therapy license request for Paula Pentico by adop-
tion of the consent agenda.
2. Deny the license request.
