The assessment was completed using the loading as described by the International Building Code, the AWWA Code, AISC 13th Edition, previous T- Mobile construction drawings dated in February 2012, an RFDS sheet provided by T- Mobile dated April 20, 2012, and information provided by others. The following antenna information was used in the evaluation: Proposed Antenna Loading Antenna Centerline Comment Elevation (9) TMBXX- 6517 -A2M 85'± Proposed / To be installed on existing (or new) mounts (6) Remote Radio Units 85'± Proposed / To be installed on existing (or new) mounts (If this information or the information contained in this report is incorrect, this report is to be considered void and the correct information is to be given to Ulteig Engineers to allow for a revised assessment and report.) Results Locally: The proposed T- Mobile antennas will replace existing T- Mobile antennas on existing antenna mounts that are attached to the legs of the water tower located at the address listed above. According to the previous T- Mobile construction drawings each existing antenna mounting bracket consists of a 4" Schedule 40 pipe spanning between (2) 8" long WT4x15.5 beam sections that are welded to the existing water tower legs. At each location there are also 3/8" gusset plates between the flange of the WT4x15.5 and the water tower legs. Two horizontal pipes are attached to each vertical 4" Schedule 40 pipe. The vertical antenna mount pipes are then attached to these horizontal mounting pipes. Refer to the previous T- Mobile construction drawings dated in Subject: T- Mobile Antenna Replacement Site: A1PO420A — Tower St. WT Address: 4741 Tower Street Prior Lake, MN 55372 Job No: 12.00851 Page 2 February 2012 for more information. A detailed structural analysis was neither requested, nor performed on the antenna mounting brackets or water tower and is not part of the scope of this report. However, the increase in loading on the brackets, as a result of installing the proposed antennas, is considered minimal and is not expected to diminish their structural integrity. It is our professional opinion that the existing antenna mounting pipes are structurally adequate to support the proposed antennas listed in the table above along with the required loading as stated in the AWWA Code. Globally: The increase in loading on the existing structure, as a result of installing the proposed antennas, is considered minimal and is not expected to diminish the structural integrity of the existing water tower structure. A structural review of the water tower itself was not performed and is not part of the scope of this report. Required Modifications No modifications are required. Comments: This report is based on the following: 1. All materials in good condition. 2. All members are in place and intact. Field verify that the existing mounting pipes are long enough for the installation of the proposed antennas. If you are aware of any information that is contrary to that which is contained in this report, or if you are aware of any defects arising from the original design, material, fabrication, or erection, you should disregard this report and contact Ulteig Engineers. T- Mobile Site Number: A1PO420A Site Name: Tower St. WT BSI Project WI 1040 Dear Mr. Hutchins: Badger State Inspection (BSI) has reviewed the revised June 19, 2012 structural assessment (which contained the supporting calculation) provided by Isaac Odland, P.E. (Reg. No. 47985) of Ulteig Engineering. BSI agrees with the report that the impact on the tower as a result of the additional loading will be minimal therefore we recommend you accept the structural report for the T- Mobile upgrade on the Hallmark water tower. If you have any questions, feel free to contact me at 715 -533 -8686. Sincerely, /1, C, A/4 Kelly C. T- Mobile Site Number: A1PO420A Site Name: Tower St. WT BSI Project WI 1040 Dear Mr. Hutchins: Badger State Inspection (BSI) has completed the final review of the T- Mobile (Rev 1) drawings dated August 30, 2012 for the above referenced antenna upgrade on the Tower 1 water tower. It appears the recommendations of the BSI initial review dated August 4, 2012 have been added to the drawings. Therefore we find that the drawings represent an installation that is acceptable to BSI. Unless the City has additional outstanding requirements, BSI recommends that the City of Prior Lake approve the drawings as submitted. A preconstruction conference with T- Mobile representatives, their contractor and subcontractor(s), the City of Prior Lake, and BSI can be scheduled to discuss the construction. It should be relayed to the contractor that BSI will be inspecting the welding, surface preparation, and coating during this project (including shop coating). The contractor will need to contact BSI to schedule both field and shop inspections. If you have any questions, feel free to contact me at 715 -533 -8686 Sincerely, Kelly C. T- Mobile Site Number: A1PO420A Site Name: Tower St. WT BSI Project WI 1040 Dear Mr. Hutchins: Thank you for providing Badger State Inspection (BSI) with the opportunity to assist the City of Prior Lake with this project. It appears that this installation consists of replacing the six (6) existing antennas with nine (9) new antennas (3 per sector). The antennas are located on the fluted column at approximately 85' RAD center. T- Mobile will also be adding six (6) new RRU's, four (4) COVP Boxes, TMA's and three (3) hybrid cables. BSI has reviewed the 7/02/2012 drawings provided by T- Mobile regarding the proposed antenna installation and we offer the following observations and recommendations to forward to T- Mobile: 1. BSI recommends that the City of Prior Lake not accept any version of the drawings until a certified structural report is included with the plans. It appears that Velocitel is responsible for the plans. BSI recommends that the final drawings be stamped and signed by a Professional Engineer registered in the State of Minnesota. 2. Under T -1 in the Site information column, please add the following under Owner's Contact: Jacob Theisen Maintenance Manager City of Prior Lake 4646 Dakota Street SE Prior Lake, MN 55372 (952) 447 -9899 3. Under S1 General Notes Add the Following: The drawings should include an additional section titled As -Built Plans and should read as follows: At the completion of the installation, T- Mobile shall be responsible for providing the City with a set of final as -built drawings. It is the contractor's responsibility to document and report any changes made during construction so the work is accurately shown on the as -built plans. 1 4. Drawing N1 Painting Specifications Add the Following to the end of the painting instructions: Antennal panel painting instructions: i. Hand scarify with 120 grit sand paper ii. Solvent wipe the antennas with Denatured alcohol iii. Prime with Sherwin Williams Pro -Cryl primer at 2.0 -4.- mils DFT iv. Finish coat with Sherwin Williams Sher -Cryl high performance Acrylic at 2.0 -4.0 mils DFT v. Color to match the existing color on the tower 5. Drawings S1 Detail B: In the RRU's Optional Mounting Note: Please modify the note: A chain mount will not be an accepted alternative for mounting the RRU's. It is an option to weld a bracket specifically for the RRU's and COVP boxes behind the tower leg. 6. Drawing S -1 General Notes and details: Provide a drawing to show a sector mounting attachment to a leg. Show details of the welds to the legs and brackets used. The drawing should include a before and after modification complete with proposed location of the TMA's, RRU,s and COVP Box as well as the antennas. Change the drawing to note all new attachments are required to be seal welded. The new mounting pipes shall have welded end caps on both ends of the pipes. 7. Drawing S -1 Please Add the Following Note: In general, where angle adapters, clamps, or any other areas of which there is metal to metal contact, provide a neoprene with a metal shim between the part and the base material to prevent excess damage to the coating and provide a moisture barrier in the event of coating damage. 8. Drawing Si: Please provide dimensions of the COVP box(s), the RRU's, and the TMA's. This may help to determining where these items can be mounted without affecting working space inside the tower. 9. Drawings RF -1 and RF -2 are all outside BSI's areas of expertise and were not reviewed. 10. It should be noted that nothing installed should interfere with normal operation or use of the tower. 11. Please provide a label on the door of T- Mobile's equipment cabinets to clearly identify T- Mobile's contact information. Labels should also be added to T- Mobile's equipment near the antennas (COVP, RU, and TMA's). This will help the owner identify T- Mobile's equipment and contact information if there becomes an issue with the equipment. Information on the label should include but is not limited to the following: a. Owner b. Site Number c. Site name d. Contact name and contact information(Corporate and Local) These recommendations should be incorporated into the construction drawings and an updated copy provided for our review, in addition to the one provided to the City of Prior Lake. Following final review and approval, a preconstruction conference with T- Mobile, their contractor and subcontractor(s), the City 2 of Prior Lake, and BSI should be scheduled to discuss the construction. Prior to construction, (if required) the contractor should provide welder's qualification certificates for our review. It should be relayed to the contractor that BSI will be inspecting the welding, surface preparation, and coating during this project (including any shop coating). The contractor will need to contact BSI to schedule both field and shop inspections. If you have any questions, feel free to contact me at 715 -533- 8686. Sincerely, Xrf. C, Kelly C.
