T'he fdllowing regulatiotxs and co�ditions canstitute s part of the application made by any inst�Uer or contractor for
the installation of utilities in any City street ri�ht-of-wxy or Gity utility easement and th�e applicant, in m�kking his ,
appiication to do such work, shal! agree ta abide by these r�sgulations and cottditions:
1. The individual obtaining tha permit shail, before cammr,ncing such work, ttdtify #he City F•nmittccr when
the work at each location is t� commence; and, whe:q requested by the Engineer, shall submit plans and ,
specsficatians. .

ra�ged edges will be permitted. Gutting shall be dpne with a concreta saw, cuttin� wheei, jack hammer,
spades or similar approved tooLs.
Backfitl material shall be suitable for highwsy embankment constraction. No peat, frozen lumgs, broken
asphali, sad, roots, or other undesirable material shall be ptnnittod as backftli. Rocks larger than six i3tc.�►es
{6") in diameter shall not be used in backfilting af trenches.
The backfill material sh,a31 be placed in layers not to exceed one foot (1') and mechanically comPacted tv
not less th�n 95 per�ent of modified ProCtor dansily.
'�'he upper six inches (6") of the backfill shall meet Minne�ota HighwaY 1�epawne�►= SPecificat�°s�s for
Class S material and shall be compaated to not iess thar► 95 percent o� modified F'roctor density.
As soon as possible after baekfill operations are eompleted, the cut bitwminaus edges of tha tranch sball be
painted with an asphalt emulsian suah as SS-1. Ymmediately thereaf�', the patchinS mate�ial shall be

property damage of not Iess than 525,000 issuod by an insurance comp�uty authoriud to do business in the
� 5tate of Minnesota on which the City is �amed as an additioaal insured party.
11. Except for the negligent acts of the City, its agents and its employees, tbe permittee shall asstune alI
liability far, and save the City, its agents and its anployees, harmless and defend sama a# its sole cast and
expense from any and �Il eiaims for damages, actions or causes of action arismg out of the worfc to be dgne

12. B7cisting Facilities - The utiliry faeillty and instaliations shall not interfere with any existing udlity facility
on #he City's right-of-way ar easement�. It is the responsibility of the appticani to catl for ttecessary

appropriate easement exists to allow the applicant ta work on private property.
