A license application has been enclosed for the addition of alcohol service from a bar. Complete this form (both sides) and submit with the applicable fee prior to scheduling your pre- opening inspection. A foodservice establishment may not operate during construction activity. Construction in the new space will be completed prior to the removal of the wall separating the existing establishment from the new area. The establishment will not be operating when this wall is removed and construction in the opened space is completed. Equipment Standards — General Requirements: Food and beverage equipment shall meet the applicable standards for one of the following: • National Sanitation Foundation (NSF). • Edison Testing Laboratories (ETL) to NSF Standards. • Underwriters Laboratory (UL) to NSF standards. • Canadian Standards Association (CSA) to NSF Standards. Equipment shall bear the NSF or equivalent sticker and manufacturer information. Wood is prohibited within a foodservice area. Wood doors or frames, wood shelving, wood cabinets and wood windows are not allowed. 2. Food Service Equipment: Table- mounted equipment that is not easily movable shall be sealed to the table or elevated on four (4) inch NSF legs. All floor mounted equipment shall be elevated on six (6) inch NSF legs or casters. Used NSF approved food and beverage equipment may not be changed or altered from its original condition. Contact Erin Gudknecht, your assigned Public Health Sanitarian, at 651 -201- 3777 to schedule an inspection for approval of any used equipment before installation. There was no used equipment identified in the installation schedule. 3. Food contact surfaces: Primary food contact surfaces (tables and counters) shall be of stainless steel construction in compliance with NSF Standard No. 2 or equivalent. Plastic laminate surfaces are not acceptable for food contact and food preparation surfaces. 4. Refrigeration: The new cold preparation table must be able to maintain potentially hazardous foods at or below 41 °F. Raised cold rail refrigeration or top air cooled units are recommended. 5. Walk -in Refrigeration: Approved flooring and base cove for the walk -in cooler or freezer could include: a. Quarry tile or ceramic tile. b. Aluminum or stainless steel floor. c. Epoxy resin (shall be approved to installation). d. Vinyl screed base provided by the manufacturer. Artisan Coffee Bistro Rem Food 130352 Page 3 2/27/13 Shelving inside walk in coolers and freezers must meet NSF International or equivalent standards and shall be approved for use in a cold environment. Effectively enclose the area above the walk -in cooler /freezer units with fixed or removable panel(s). The area above the walk in cooler /freezer may not be used for storage. Provide access and ventilation for equipment as recommended by manufacturer. Model walk in cooler approved: Nor -Lake Kold Locker Floor & interior base cove: Manufacturer supplied smooth aluminum panels and stainless steel cove. Exterior base cove: quarry tile. Install a sufficient number of vapor proof light fixtures in the walk -in cooler and/or freezer to provide a minimum of 10 foot candles measured at 30 inches above the floor. Condensate from interior walk -in refrigeration equipment shall be drained to a floor drain located outside of the unit, or the unit shall be equipped with an evaporator pan. The new walk -in cooler will be used for the storage of kegs in addition to food supplies. Keg storage is typically separated from food storage as they are considered sources of contamination. This storage area must be maintained clean and the food supplies stored in this refrigeration must be protected through distance or physical barrier. If food is found to be contaminated during inspections, removal and disposal may be required. 6. Ventilation System: A Superior VSE48 -9 Type I hood is provided for the new broiler, salamander, oven, and gas fueled steam table in the new kitchen area. All rooms shall have sufficient tempered make -up air and exhaust ventilation to keep them free of excessive steam, condensation, vapors, obnoxious or disagreeable odors, smoke and fumes. Sufficient tempered make - up air (at least 55° F) shall be provided and interlocked with ventilation equipment. All open sides of a canopy hood shall overhang equipment by at least six (6) inches. Exhaust ventilation systems shall be designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, walls, ceiling, linens and single - service articles. Ventilation hoods shall provide at 50 foot - candles of shielded light at the working surfaces. Exposed utility service lines and pipes shall not be installed directly on the walls or floor, except: a. quick disconnect gas hoses approved by the American Gas Association or NSF International, and b. flexible cords and caps for commercial cooking equipment on casters, listed by Underwriter' s Laboratory. Artisan Coffee Bistro Rem Food 130352 Page 4 2/27/13 7. Hand Sink: An NSF International listed hand sink is provided at the new bar. Splash guards must be installed on this sink to protect clean items from contamination at the adjacent table and shelves. The plan has been modified to add one NSF International listed hand sink in the transition area between the new kitchen area and the new bus station. Accessible hand sinks must be provided in each food /beverage preparation and warewashing area. If handwashing frequency is found to be an issue, additional hand sinks may be required. This will be monitored during inspections by the assigned Public Health Sanitarian. Each handwashing sink shall be provided with hot and cold water through a mixing valve or combination faucet. Self - closing or metering faucet shall be designed to provide a flow of water for at least 15 seconds without the need to reactivate the faucet. 8. Warewashing: In response to questions related to the additional hot water demands and the adequacy of the existing water heater, the plans were modified to add an undercounter dish machine in the existing warewashing area. The review of this installation area indicated a need for an accessible hand sink for this process area. In response, the dish machine was then removed from the installation plan and thus removal from consideration. Hand wash facility accessibility and hot water supply will continue to be evaluated during routine inspections of this establishment. 9. Dump Sink: The plan has been modified to add a dump sink at the new bar. The plan has been modified to add water glass filling equipment at the bus station so that the sink at this location can be reserved as a dump sink. A ware washing sink, food and beverage preparation sink, or handwashing sink must not be used as a dump sink. 10. Floors: Floors in kitchens, bars, other rooms where food is stored, prepared or washed, employee dressing or locker rooms, toilet rooms and janitorial rooms shall be smooth, non - absorbent, durable and easily cleanable. See the approved finish schedule listed above section one. Artisan Coffee Bistro Rem Food 130352 Page 5 2/27/13 Where a wall meets the floor it must be properly coved within any foodservice area. A four inch integral base cove shall be installed at all floor /wall junctions. Correct Method Incorrect Method Non -slip tile may not be located underneath equipment. 11. Plumbing: A plumbing plan approval letter has not been received at this time. Provide at the opening inspection evidence that the plumbing system has been inspected and approval given by a representative from the Department of Labor and Industry (DLI). A separate on -site inspection will be conducted by a plumbing inspector to determine compliance with the Minnesota Plumbing Code. All plumbing equipment shall be installed in accordance with the Minnesota Plumbing Code for a commercial establishment. All pipe chases that pass through walls shall be tightly sealed and covered. All utility pipes shall be enclosed in walls or ceiling. Utility lines must not be placed on the floor and must be installed at least 6" off the floor. Wye adapters are not permitted. Indirect waste pipes shall not discharge into hand sinks, prep sinks or three - compartment sinks. Telltale drains are required for food service sinks (3 compartment, food preparation and dump sinks) or as required during the plumbing plan review. Artisan Coffee Bistro Rem Food 130352 Page 6 2/27/13 All hot water generating equipment (water heaters) shall be of adequate capacity to meet the needs of the anticipated demand of the establishment. A commercial 50 gallon Rheem water heater is existing and is described as meeting the needs of the establishment. The additional plumbing fixtures added to the establishment are calculated as having an additional 12 gallons of hot water demand each hour. 12. Lighting: Food preparation areas in which food or beverages are prepared, utensils are washed shall provide a minimum of 50 foot - candles of shielded light measured 30 inches above the floor. Bar lighting shall provide at least 20 foot candles measured 30 inches off the floor. 13. Employee Lockers, Restrooms and Break areas: Lockers or other suitable facilities shall be provided for the orderly storage of employee's clothing and other possessions. An office and employee area furnished with lockers will be added to the back area of the establishment for the storage of personal belongings. 14. Bar, Wait station, and Bus station: The interior of the bar shall be finished with approved flooring, base cove and wall materials. See the approved finish schedule listed above section one. The underside of a wood bar top must be properly finished. The bar top finish will be plastic laminate. The beer pouring area is finished with a stainless steel. Cold plates inside ice bins shall be constructed integrally with the bin. Waste lines form pop guns shall not pass through the ice. The bus station counter top will be solid surface. This counter will be wall mounted and storage below is intended for carts with dirty dishes. 15. From what I know it sounds like we have the following topics to discuss: 1. SAC Units for Artisan Cafe - We believe that there is one left. We are checking that with the Met. Council. I want to see what other options there might be as well. 2. Roof- Bob tells me that he is recommending insulation along the bottom of the cord rather than immediately under the roof to protect your roof investment and reduce long term heating and cooling costs. I suspect this has a cost though to. Feel free to talk to Bob Hutchins on this. 3. Exterior Wall Material- I know an elastomeric paint has been proposed for the exterior walls over the block. I believe our design ordinance would call for a siding which mimics the EFIS you placed on the South Lake Village. It seems to me that paint over block would not be able to do that even if it is textured but I also know that EFIS is expensive. A less costly alternative would be to replace the wood on the exterior but I believe that would require amendment of the roof line to provide a gutter so that water cannot find its way down the wall under the exterior surface. lam sorry for my lack of availability so I wanted to share a few thoughts before I have to run off. Dan and Bob are available today. They can be reached at 952 - 447 -9800. 1 Leachate: Liquid that has originated within or has percolated through refuse or through the protective cover of a landfill. Local Government Unit: Any entity within the area served by the MDS that is authorized to issue building permits, sewer connection permits or equivalent. Local SAC: The actual total fee paid by the property owners to the Local Government Unit, which may include Metropolitan Council SAC passed on by the Local Government Unit and any local add -on charges. These fees vary widely between Local Government Unit's and could add substantially to the total SAC costs of a property or business owner. Look -Back Period: The period of time prior to a Determination that MCES looks at to determine applicable Credits when a SAC Determination occurs. The Look -Back Period is the seven full years prior to the new SAC Determination or permit issuance and that part of the current year up to the Determination or permit issuance. For example, if a permit is issued in July 2012, the Look -Back Period will go back to January 1, 2005 (seven calendar years plus the portion of the year 2012). Maximum Demand: For Commercial properties it is the highest capacity demand in SAC units within the Look -Back Period. For example, if in the Look -Back Period there had been a 4 -SAC, 7 -SAC, and 5 -SAC demand, the Maximum Demand in the Look -Back Period is the 7 -SAC Determination. See Section 5.2.2 for further details. For Permitted Industrial Users it is the highest recognized demand determined during the Look -Back Period by either a third year Capacity Demand Review or a special review that resulted in additional SAC being paid after the most recent Capacity Demand Review. Metropolitan Area: The seven county Metropolitan Area over which the Council has statutory jurisdiction pursuant to Minnesota Statutes section 473.121, as may be amended from time to time. Hanover and Rockford (in Hennepin County), New Prague (in Scott County), and Northfield (in Dakota County) are currently excluded from the Metropolitan Area. Metropolitan Council Environmental Services (MCES): The division of the Metropolitan Council (Council) responsible for the regional wastewater conveyance and treatment operations, industrial wastewater management, water quality monitoring and water supply planning. Metropolitan Disposal System (MDS): All facilities (e.g. pipes, meters, lift stations or wastewater treatment plants) that are part of MCES' sewer service area and owned by the Council. These are generally the wastewater treatment plants and related conveyance systems in the Metropolitan Area. Net Credit: Prior to 2010, this was SAC Credit over the amount needed on a Site being redeveloped. These were allowed to be accumulated throughout the Local Government Unit (City -Wide Credits) or retained for the future use on a Site (Site - Specific) through December 31, 2009. The original Quizno's space had been charged 1 SAC unit after a credit given for the original retail use. Quizno's project was started in December 2004. The unoccupied Quizno's space has 1 SAC credit for previous restaurant use. Any extra SAC credit, above a retail use SAC charge within the building, can be used in any tenant space. A new more intense tenant use, above and beyond a retail space charge, may use any remaining SAC credit in the building. There had been one site specific SAC unit left from the original Laundromat.
