Dear Mr. Anderson:
It has come to my attention that the above referenced connection
was made to the Metropolitan Waste Control Commission 's (MWCC)
interceptor without notifying the MWCC in accordance with the
permit requirements.
Please be aware that should this connection fail, the MWCC will
look to the City to correct the failure.

RE: Permit for Connection To or Use of MWCC Facilities
Permit Number 1241
Dear Mr. Raaen:
This letter is to inform you that the Metropolitan Waste Control
commission has approved your application for Permit for Connection
To or Use of Commission Facilities. Enclosed is your approved MWCC
Permit Number 1241 for the connection of a 4" PVC sanitary sewer to
MWCC's Interceptor 70-20-01 near Pleasant street and Lakeside
Avenue in the city of Prior Lake (see enclosed plan sheet). Also
enclosed is an extra copy of the approved permit. This copy should
be provided to the contractor who will be making the connection so
that s/he is aware of the MWCC's requirements.
Please note that the connection shall be made in accordance with
Standard Detail SC-5 which is attached to and made a part of
Connection Permit No. 1241.
It is required that the city contact Mr. Wally Wessels' office at
telephone number 612-772-2585 (7:30 a.m. to 4:00 p.m.) and make
arrangements to have an inspector on site at the time the connec-
tion is being made. A minimum 48-hour notice should be given.
Please refer to the permit index number when requesting an
inspection.
PLEASE NOTE: If the Commission's inspector is not notified prior
to installation of the connection. the applicant will be reauired
to pay all costs associated with re-excavation and inspection.

PERMIT
It has been determined that Applicant has adopted a comprehensive
sewer plan, has submitted it to Commission as required by law, and
that the proposed connection to, or use of, Commission facilities'
conforms to the adopted comprehensive plan or adopted revisions
thereof.
Permission is hereby granted to Applicant for connection to, or use
of, Commission facilities in accordance with the description provided
by Applicant. This permit is issued under the following terms and
conditions:

Minnesota Pollution Control Agency or the necessity of securing other
permits and fulfilling requirements of other governmental units.
3. The Commission's issuance of this permit does not constitute
a certification that the project design and construction proposed by
Applicant meet engineering standards. Applicant remains responsible
for design and construction, including safety, for the project.
Applicant's attention is specifically directed to the confined space
rules adopted by the Minnesota Department of Labor and Industry, OSHA
Regulations, Parts 5202.100 through 5205.1040 and 5207.0300.
4. This permit shall be null and void if the connection
proposed by Applicant is not made within 90 days from the date of
issuance.
5. This permit is subject to modification or revocation and may
be suspended at any time for failure to comply with its terms.
