INTRODUCTION:
During the course of construction for Project 93-02, additional
work beyond the Contract was done by the Contractor. Change
Order No. 1 for this additional work has been prepared and
enclosed in the Agenda packet for Council approval.
BACKGROUND:
The Contractor, S. M. Hentges & Sons, Inc., has performed
additional work which was not included in the original contract.
The additional work is described and itemized costs are shown on
the enclosed Change Order form. A total of two (2) additional
items are described with the total net cost of the Change Order
being $16,267.80.
Item # 1 is extra work associated with the City's future watermain
system. The installation of the valve was required for testing
purposes and to isolate the residents of the Brougham Boulevard
cul-de-sac. The new valve will allow for the future extension of
the 16" watermain along CSAH 21 from Carriage Hills Parkway
north to CSAH 42 without shutting off the water service to the
residents of Brougham Boulevard cul-de-sac. The cost is for the
valve itself, installation was done at no charge to the City by the
Contractor.
Item #2 is extra work associated with the removal of extra
unsuitable sub grade material. The proposed street was constructed
through an old low area where topsoil had accumulated to a
thickness of eight feet. Plans for the project were based on aerial
topography, and not soil borings. The depth of the unsuitable
sub grade material varied from 1.75 feet at station 5+00 to 8.75 feet
at station 6+25 to 7.00 feet at station 7+00 to 4.00 feet at station

8+00. The top 1.00 feet of topsoil removed was completed as part
of the normal topsoil removal included in the original contract.
The total amount of this Change Order is $16,267.80 with the
original contract amount being $512,513.30. The addition of
Change Order No. 1 represents a 3.2% increase in the original
contract amount and the contract amount total is now $528,801.10.
ALTERNATIVES:
The alternatives are as follows:
1. Approve a motion authorizing Change Order No. 1 in the
amount of$16,267.80 to S. M. Hentges & Sons, Inc.
2. Disapprove the motion for a specific reason.
3. Table this item for a specific reason.
RECOMMENDATION: The recommendation is to approve Change Order No.1 as the
contractor has completed the work and City staff feels the
additional cost is acceptable. The work was required to complete
the project.
FINANCIAL IMPACT: This amount will be paid from the Construction Fund with
Municipal State Aid dollars, and will be included with this month's
construction voucher. Sufficient funds exist to pay for the
additional cost from this Change Order.
ACTION REQUIRED: Make a motion and a second as part of the Consent Agenda to
approve Change Order No. 1 for Project 93-02 authorizing the
payment of$ ,267. .

DESCRIPTION:
Installation of a 16" butterfly valve for the watermain crossing east of CSAH 21, and the removal
of 4417 cubic yards of unsuitable subgrade materials that were encountered during grading
operations.
PURPOSE OF CHANGE ORDER NO.1:
The purpose of Change Order No. 1 is to perform the work described below that was not included
in the BID.
ITEM NO.1
The installation of a 16" butterfly valve. The lump sum cost for materials and labor is
$1,250.00.
ITEM NO.2
The removal of 4417 cubic yards of unsuitable subgrade materials per Supplemental
Agreement No.1. The total cost with a contract unit price of $3.40 per cubic yard for all
labor and equipment required to remove the unsuitable subgrade material is $15,017.80.
