5. Project Notes:
a. Original precon was held March 28, 2016. SAC Wireless hired Vinco Inc. as GC. CD's
called for white coax/taped white coax/jumpers. Vinco had not bid this item. SACW did not
accept pricing from Vinco on taping and chose a new GC—Mill City.
b. Second precon held May 18, 2016 with Mill City.
c. City identified old AT&T mounts to be removed as part of project—Site to be restored to pre-
existing condition
d. Mill City left site June 10-24, stating wet conditions.
e. Restoration extended over one month past construction completion.
f. Base plate(s)for abandoned AT&T mounts were approved to remain per Nick Zurbey. Stand
offs were ground off and surface was restored. This prevented having to work directly on the
tank leg(s).
g. Close-out was held until final restoration was approved (September 19, 2016).

Dear Mr. Humphrey:
In accordance with the request of the City, we have reviewed the plans submitted by SAC Wireless
dated December 09, 2015, referencing the construction of the proposed AT&T Mobility Project at the
Tower Street Water Tower in the City of Prior Lake and have the following comments for your review:
1. Tenant needs to verify with the City that the proposed additional antennas and equipment are within the
lease space on the water tower and quantities as specified are in the lease documents.
2. Note: This project and all aspects of construction will need to be discussed at a preconstruction meeting
prior to commencement of project construction.
Based on our review and the above information as provided by SAC Wireless, it is our opinion the City
of Prior Lake can proceed with approval of this phase of the project, as the remaining comments are
minor in nature and must be addressed at the preconstruction meeting. Following approval by the City,
a pre-construction meeting must be scheduled by the tenant to include the City, SEH, the contractor
and AT&T representatives prior to commencement of project construction.

• Numerous connection details of large-scale structures are unobtainable and are omitted from the structural analysis .
This includes, but is not limited to: bolts, welds, flanges, and plates. These connections are considered adequate
and are therefore neglected from the analysis. In addition, in the absence of building plans, many wall, floor, and
ceiling constructions can only be determined from observable field data and are supplemented by best judgment
and experience.
• Antennas, dishes, feedlines, and any other such appurtenances are assumed adequate through manufacturer
testing. No analysis is provided for the structural strength or stability of these items unless otherwise specified.
• Equipment mounting systems are assumed structurally sound unless specifically called for in the analysis .
• Soil conditions and foundations are not considered unless specified in the analysis and have no deterioration or
defects. For sites located on a building, only local effects of the equipment is considered unless otherwise
specified. The overall structure of the building and its foundation are assumed to be unaffected by the telecom
equipment.
• Any changes or differences to the site or site plans at any time prior to installation must be brought to the attention
of Westchester immediately.
