Second Notice
RE: Captain Jacks Restaurant, 3950 Green Heights Tr. Prior Lake MN
Dear Mr. Kinsella,
On February 24, 2006, during an inspection of a fire sprinkler system repair, It was noted
several door locks had been replaced or altered and are now not in compliance with the
currant Building Code and Fire code. The locked doors may block escape in the event of
a fire and requires prompt attention, Since that time, I have called you on the matter twice
with out any resolution.
Areas that exceed an occupant load 100 shall have panic hardware on all required exit
doors. (IFC 2003 sec. 1 008.1.9) All other required exits doors require hardware that can
be unlocked from the interior by one hand, easy to grasp and without any pinching or
twisting motions to operate. (SBC 134.0442 subp.9) Doors that are not in compliance are:
Front east facing main entrance. The south facing exit from the bar area. The west facing
exit, at the top of the stairs, The east facing exit in the lower area.
Installation of the correct door locks and latches must be completed by May 17,2006

Dear Mr. Kinsella,
During the inspection for the fire sprinkler repair April 29, 2005 several noncompliant items were noted with
the existing system. A few of the items noted are.
1. Blocking the access to the Fire Department connection.
2. The deep fat fryer was moved from under the fire suppression system designed for fryers,
3. Fire alarms were not distributed through out the building.
4. Fire sprinklers do not properly protect several areas. IE: The crawl space, deck-cooking area, screened
porch area etc, In other areas, added decoration has interrupted the sprinkler head protection area.
The City of Prior Lake would like your cooperation in resolving these items in a timely manor. Please call to
schedule an inspection time with a licensed fire sprinkler contractor and with myselfto fully review the fire
sprinkler system by May 27,2005. When the deficiencies are identified, a plan and time table for correcting the
noncompliant fire suppression system will be established. The City will review the plans for compliance and
issue any required permits.
