As requested, site visits were made on July 18, 19, 21, 27. 2006 &
Augus~ 24, 2006 to evaluate ptaceme0t of 3t~u~tural steel and decking
~ithin a 3 story commercial buildlng under construction. Construction
and structural plans dated January 31, 2006 as prepared by David P.
Morris P,A. Structural Engineering and Quigley Architects. were
available tor review at the site on 7/19/06.
7/'...3/06 (2:00 P.M.) &. 7/19/06 (2:40 P,M.):
Observations were made on 16 vertical I-beam columns placed at
locations integral with the perimeter foundation and interior pads at
the first floor of the structure. The I-beams were installed without
shop attached base plates, using temporary field welds completed at
the bottom of the I-beam and a separate bearing plate at each pad
location. The I-beam to plate fillet welds.. averaged 4" in length. on
a minimum of 3 to 4 web faces. '",ith platt~s s~cH!'ed to firmly anchor
bolts embedded within the loadbearing pads. The application of a non-
shrink grout between base plates and the pad bearing surface had yet
to be completed. Column to pad lateral alignment was acceptable. with
the exception of a column. 3rd in from the northeast corner which
indicated eccentric loading.
Horizontal structural beam segments had been completed to the 2nd
floor and partial at the 3rd floor. with attachment between the I-beam
columns and within wall pockets at the north CMU wall. All beams had
sufficient bearing surface at wall bond b~am pldtes and were securely
welded. The beams were supplied, where required, with shop fabricated
mid-section web stiffners and appropriate anchor plates and vertical
"T" bearing attachments were bolted to columns. Beam to column
attachment employed break-off tension control type ASTM A-325 bolts.'
The A-325 bolts were inplace within the first floor level and properly
tensioned, as the bolt ends were snapped off. Bolts at the 2nd & 3rd
floors were only partially installed at the time of the inspection.
Visual examination of the bar joists indicated that they were erected
with sufficient bearing provided at the ends of the joists and welded
with 1/4 inch fillet welds, averaging 2 3/4" to 3" long on each side
of ~he bearing end at beam connections. Upper and lower bridging

braces we~e installed between each bar joist span, with all
connections secured with welds.
Perimeter angle plates providing bearing support and attachment for
the floor deck, were fillet welded at exterior beams and at
north/south beam segments. All welds wer~ satisfactory, with
acceptable genetration, with no porosity, 8r under cutting observed.
The 1" - 24 gauge type CSV Formdeck had been completed at the 2nd
level, and was secured to bar joists with 5/3" puddle welds at
approximately 31 1/2" O.C. using Nucor welding washers, with panel end
seams welded at perimeter plates at an average of 18 inches O.C. All
welds were acceptable, with satisfactory penetration and retention.
7/21/06 (9:05 A.M.) I; 7/27/06 (12:20 P.M.):
Inspected completed placement of 3rd tloor decking and respective
welds, all work was satisfactory. Discussed floor and roof deck weld
specifications, and previously referencp.d eccentric column loading
with project structural engineer. As no cable restraints or other
support had been installed for the building frame work, it was
recommended that all column base plates be fully welded and the non-
shrink grout be applied prier to the topping pour scheduled for
7/25/06. This recommendation was communicated to the building
contractor on 7/24/06. Observations made on 7/27/06 indicated that
the topping for each floor had been completed and the columns welded
full length of each web at the I-beam and base plate juncture, with
all welds exhibiting acceptable penetration and coverage at each
segment. The non-sh~ink grout had also been applied between the
column base plates and the pad bearing surface.
8/24/06 (11:05 A.M.):
A final inspection indicated that all A-325 Structural bolts had been
installed and secured throughout th~ st~ucture. Observations were
a 1 S 0 made 0 n t he ins tall at i on 0 f t he 1 1/ 2" - 2 0 Ga. i n term e d i ate rib
metal roof deck. As access to the roof surface was not was available,
all observations' were made at the underside surface from the 3rd
floor. The decking appeared to be secured by 5/8" inch puddle welds
with a 36/4 weld pattern. Although we were not able to evaluate the
retention of the welds, only minimal burn through was noted in
isolated areas. The roof panel side laps were not secured by Tex
screws between the midspan supports, although this condition may be
acceptable based on bar joist spacing of 5 feet or less, we
recommended it be verified by the project engineer.
With the-exception of areas noted for review, our inspections indicate
that the aforementioned work was satisfactory and appropriate for the
application and i~ compliance with the project plans or accepted
engineering standards.

As requested, a site visits we~e conducted on Ma~ch 27: 28, 29, 30,
2006 to evaluate suhgrade soils and test engineered fill 'thin a
commerc::ial building site. The subsurface soils exploration
and constructions plans were not available for review during the
inspection, therfore referenced elevations or demensions are
estimates based on visual observations.
3/27/06 (10:00 A.M. & 2:34 P.M.)
A prelimenary assessment was made in the A.M. for the possible reuse
of undocumented on site fill soils for construction of the building
pad. A 3 foot test pit ecvavated nea~ the west footinq line
indiciilted subgrade soils comprised primiilrily of a loamy sand and
gravel with isolated pockets of moist cohesive soils. The observed
soils were generally free of organics and unsuitable debris," and
would be acceptable as engineered fill, ~t is our understanding that
the bulk of the fill requirements will consist of imported granular
mixed with selected en site seils; Th~ soil moisturB content should
be moniterd durinq compaction to avoid a above optimum and unstable
condition, where the moist cohesive soils present a large percentage
of the fill zone.
Observations were made in the P.M. within a 8 to 9 foot excavation
completed at the northwest corner of the buildinq pad. The excavator
had exposed a CMU course intergal ~ith footings and a basement slab,
from a previous structure, covering the excavation zone of
appproximately 18 x 35 feet. The masonrycomponets were removed to
'the underlying natural soils consisting of a COarse sand with gravel.
To evaluate the relat.ive strength of t.he soils, a 5/8" diameter
smooth probe and 1 3/4" fluted Were used to penetrate 50ils at random
at the excavation base. With full body ~ei9ht applied, the
probes indicated acceptable strength at depth.
To assess the relative loadbearing capacity of the natural suhgrade
soils, dynamic Cone nenatrometer tests were conducted ~ithin the
base of the completed excavation and at 2 test pits near the mid-

point of the p~oposed build area. The dynamic Cone penetrometer
(DCP). useg & eight kiloq~am (17.6 pouud) hammer to drive a 20
millimeter diameter conical point into the soil-, with thepenetration
recorded in millimeters per blow. From the DCP readinqs, a p$r blow,
penetration averaqe Was recorded and a N-va!ue derived. This N-
~alue, is an empirical relationship we haue established to equate the
dynamic cone data to the m9re familiar blows per toot (BPF) standard
penetration soil boring test.

A site visit was made in the A.M. to evaluate subgrade soils and the
removal of additional foundation and slab materials within the
southwest section of the building pad. As the excavation was
expanded to the east, a septic tank ~as uncovered near the mid-point
of the pad. The cont~actor had effectively removed deb~is Dr
unsuitable materials to a depth of approximately B to 9 feet.
Observed soils within the excavation consisted of a glacial outwash
coar:se :sand, full depth of the excava.tion. The excavation was
subsequently approved for backfill.
By the P.M. the excavation had been completed from full width at
th@ mid-point to a 12 foot wide section at the northeast corner of
the proposed build area. The average e~cavation depth; as measured
at the north pad line, was a.p~roximately 7 feet. Soils at the base
of the excavation consisted of a moist cl~yey sand. Probes conducted
at random indioated an acceptable 12 to 18 inches of penetration.
Before leavinq the site, fill placement had been completed to near
the mid-point of the pad, and tested with the dynamic cone
penetrometer.

Pad eX~avation in the P.M. had un~overed a 2nd septic tank, lQQ.ted
app~oximately 60 to 65 feet east of the west pad line within the
north side of the excavation. All unsuitable materials had been
removed to approximately 13 feet, with granular soils full depth ot
the excavation. Also removed were soils within a 25' x 15' section
at the southeast corner of the pad. Excavated soils consisted of a
upper soil layer of undocument loarny till,' al\d a underlying zone of
dark organic peaty soils. At approximately 11 feet, as measured from
top of curb, soil at the exoavation base consisted of a dark brown
silty clay loam. A handauqer probe indicated that the soils were
stiff and of acceptable strength.
3/30/06 (8:39 A.M.)
The east section of the ex~avation approved cn 3/29/06 had been
backfilled and was tested for bearing capacity, ~ith the dynamic cone

supports with 5/8" puddle welds in a 36/4 pattern, with end seam
attachment at east/west wall channels in a 36/7 configuration. Panel
side lap welds at the north/south wall channels were placed at an
average of 5 to 6 inches. All welds indicated acceptable penetration
and retention. A l' wide filler panel installed at the north wall,
was well secured at the inner side lap with #10 Tex screws at l' on
center, however, threaded fasteners were not placed at the panel side
laps between the mid-span supports within other panel sections of the
roof deck (reference report of 8/24/06).
Near the mid-point at the north wall, a deck panel (2nd from the wall)
and spanning 5 bar joists, was observed with a fracture extending 14
3/4' of the 17' panel. The fracture, near the base of a bearing rib,
appeared much like a stress area created by forming and subsequent
work hardening of the metal. Although the fracture fully penetrated
the metal, only a section at the east end spanning a 3 foot bar joist
section, indicated separation not exceeding 1/8".
Installation of window lintels was nearing completion, utilizing
custom made units fabricated from 4" x 6" and 4" x 8" box channel,
with an attached 3" x 5" shop welded flange. The units were fillet
welded at each end, at the exterior face on the vertical and
horizontal axis, to the 2" x 6" vertical metal studs of the window
openings. It is our understanding that welds will also be placed at
the interior junctures of the lintels as part of the completed
installation. Isolated lintel units at the building west side, were
also retained with the addit.ion of "L" brackets to the studs for

s~pport of the lintels, attached by threaded fasteners and welds. The
quality of the welds were above average, unbroken beads providing even
~overage at each mating surface, with good penetration, and no
porosity. In addition, all weld~ wer~ painted for corrosion
protection.
On September 18, 2006 our observations were conveyed by phone to the
project structural engineer, Mr. David P. Morris. His recommendation
for repair of the roof panel fracture would include a overlay of a
panel section, appropriately secured by welds or possibly Tex screws.
Based on our conversation, the detail for lintel installation
recommended "L" brackets, although fillet welds may be acceptable
based on the quality and coverage of welds attained at the lintel and
frame work junctures. He plans to inspect and advise on these issues.
With the exception of areas noted for review, our inspections indicate
that the aforementioned work was satisfactory and appropriate for the
application and in compliance with the project plans or accepted
engineeLing standards.
